Shared Lives Service
How do I find out more?
To make a request for Shared Lives you can:
- Get in touch by contacting the Customer Service Centre for Adults and Communities on 0116 305 0004
- Talk to your named social worker if you already have one, who can make a referral to our service
An assessment of your needs will then be carried out to see if you are eligible for a Shared Lives Service.
