Rehabilitation (Reablement)We aim to help people to live as independently as they can. If following an assessment, the need for support in the home is identified, Homecare Assessment and Reablement teams (HART) will work with referred people on a short-term basis of up to six weeks, with the aim of assisting them to regain and maximise their independence. At the end of this period the person's needs will be reviewed and any ongoing care needs will be re-commissioned through an Independent provider. It should be noted that in many cases, people are enabled to regain full independence and do not require ongoing care
We offer a similar service in a residential care home setting. This is for people who have experienced a significant change, either due to illness or disability and need a short period (of up to six weeks) in a 24-hour care setting to regain skills of daily living. Again, the aim is to assist the person to return to living safely in their own home. There is no charge for this service.
These services can be accessed by contacting your local Social Care office. An assessment will be carried out and services provided if the needs identified meet the eligibility criteria for services.
