Assessment

To find out the best way to help you a worker can visit to talk with you about what help you need. This is called an assessment.

We will write down what help you need.  We will  talk to you and people who know you - any people you would like in your assessment, for example your parents and family carers. But the most important person we talk to is you.
When your worker is writing your assessment they will talk to you about it and give you a copy of it.

We will ask you about things like:
  • The help you already get
  • The things you can and cannot do for yourself
  • Where you want to live and who you want to live with
  • What you choose to do in the day time and evenings
  • What your views, needs and wishes are
  • How we can help you

You can also have a Person Centred Plan.
A person centred plan puts the person at the centre of planning help and support for their lives.
  • Listening to and learning about what people want from their lives
  • Helping people to think about what they want now and in the future
  • Family, friends, professionals and services working together with the person to make this happen
When your assessment is done, we will try to make sure you get the help you need.
