What happens when you need Social Care Services?

Assessments will be done by a trained member of staff, usually in your own home. It will be done in a way that allows you to be fully involved and encourages you to take part in any decisions that are made. If you have children, and it is appropriate, we would want to involve them in the assessment.

The assessment will take account of your individual needs (for example, ethnic and cultural background), and, if necessary, we will provide help in your first language.

During the assessment a member of staff will meet with you and any family you have, or other people who are supporting you.

Where other people like health visitors, doctors or teachers are already helping you and your family, it is likely that we will talk to them too. We will discuss this with you.

The information you give us is usually held in confidence. If we need to discuss this information with anyone else, we will normally ask for your permission. Sometimes, by law we must share information with other organisations, for example, information about protecting children and vulnerable adults. If this is the case, we will explain what happens.

You will receive a written copy of the assessment. This will say what you feel you need, and what services may be provided by us or other organisations.

You can tell us if you do not agree with any parts of your assessment. We will look at this with you.
