If you wish to look at your Social Care Records you will need to write to either
the Director of Social Care and Health if you are an adult receiving services or the Director of Children
and Young People Service if you are a young person or your records were compiled when you were under
18, the address for either request is: County Hall Glenfield Leicester LE3 8RL When requesting your records you need to give as much information
as possible which will help us locate your records a minimum requirement would be: Name,
Previous Name, Current Address, Previous Address, Date of Birth, Services receiving/received (for example looked after child, receiving homecare) If
you are acting as an agent on behalf of another person please also give your name and address. All people
requesting access to their information will be asked for proof of identity. If
you would like this information on tape, in Braille or in another
language
