For advice and support on hospitalisation and NHS services contact your local health services. They can provide information advice, support, and specialist services/equipment to help to manage your medical condition
If you are in hospital and are due to be discharged there is a process within the hospital that identifies people who may need additional support to return to their home. If you are concerned about how you will manage at home when you leave hospital, there is a range of advice and support. For example:

- advocacy
