
Managing a Long Term Illness
- A long term illness/condition is an illness that cannot be cured but can be managed through medication and / or therapy and support
- There is no definitive list of long term illnesses, but conditions such as dementia, diabetes, asthma, coronary heart disease, chronic obstructive pulmonary disease (COPD) and mental health issues would all be included
- People with long term illnesses can find themselves having to deal with both the National Health Service (NHS) and local social care services
- A lot of work is going on to try to improve delivery of services to people with long term illnesses, so that they experience a seamless service
- If you would like to find out about the support you could get to help you manage your long term illness and to live life to the full, we will work with you to do an assessment
- While we are doing the assessment we will check to see if you are eligible for support and also whether your support should be paid for by the NHS or social care services
