
Carer’s Assessment
- Adult carers who are providing substantial amounts of care on a regular basis for a person aged 18 years and over have a right to a carer’s assessment
- Young people caring for an adult family member will receive an assessment regardless of the amount of hours per week they are providing care
- A carer’s assessment is a set of questions that will identify how caring impacts your life
- It helps us work out what support we might be able to offer you
- We can carry out a carer’s assessment even when the person you care for refuses help or an assessment for themselves
- You can do a self assessment (Impact of Caring form)and tell us what support you think might be helpful to help you continue in your caring role and to make your life easier
- You can either:
- Fill in the online Impact of Caring Form or
- Print the Impact of Caring Form (556 kb .doc), complete it and return it to us
- Alternatively, please contact us to arrange for a member of staff to work with you to do an assessment.
