How do I Apply for a Badge?
Badges are issued for a period of 3 years. Application forms for renewal are sent out automatically about 6 weeks before your badge is due to expire.
Please allow at least 3 weeks for processing your application for a badge. If we decide you should attend an independent mobility assessment, the processing time is likely to be significantly longer.
This also includes applicants who have previously held a badge.
Only one badge will be issued per person.

You submit the form and your photograph online, but will still need to send us by post the required proofs of identity and address (and proof of eligibility if appropriate). Our address is below. Most people will find it easiest to complete the form online, but you can instead print and complete an Application Form by hand and post it to us.
