To apply for the English National Concessionary Travel Scheme pass, or for travel vouchers, you need to complete an application form and post the form to us. The forms give details of the concessions available and the return address.  If you are of eligible age or over and disabled, it is best to apply via the disabled persons’ scheme.

Please note that the applications below are in PDF format which is read only and you will need to print out a copy to fill in and return to the address provided, enclosing a photograph and if you are applying for an older person's bus pass proof of eligibity e.g a copy of your passport, birth certificate or driving licence. If you are applying for a disabled persons bus pass you will need to provide evidence of your disability as listed on the application form.

From 6th April 2010, the statutory age of eligibility for an older person's travel pass changed in line with changes to the women’s qualifying age for state pension.
