APPENDICES FOR THE STATUTORY ASSESSMENT PROCESS

During the Statutory Assessment process a variety of information is collated and presented to the LEA as an appendix. These are as follows:
Additional Information Sheet

APPENDIX B - EDUCATIONAL ADVICE
(Completed by the school and / or Specialist Teaching Service) - Word document (37 kb)

APPENDIX C - MEDICAL ADVICE
