CAF is a process for undertaking a common assessment, to help practitioners gather and understand information about the needs and strengths of the child, based on discussions with the child, their family and other practitioners as appropriate.

You can download the template and complete the new CAF form online. However, for security purposes you are unable to save the form once you’ve entered data so either fill it in and then print it off or print it and write onto it. Hard copies will be available from us in the near future.

Please post a copy of the completed form to the appropriate CAF Team, give the parents / guardians / young person a copy and hold onto the original for yourself.
