Terms of Reference
Roles and Responsibilities
On behalf of the SEN and Disability Strategy Group the Aiming High Implementation Group will be responsible for ensuring that services meet the Aiming High for Disabled Children and Families core offer
Consistent with Aiming High the group will be responsible for:
- Developing and meeting the Aiming High for Disabled Children and Families core offer
- The transformation, planning and development of ‘short break’ provision for disabled children and their families
- Establishing minimum standards for information, transparency, participation and assessment
- Development of personalised, individual budgets for flexible packages of care
- Establishing data collection to support local authority and PCT planning processes
- Proactively seeking the views and active involvement of disabled children and young people and their parents/carers
- (In liaison with Transitions Group) Improving transition arrangements
- Supporting child care accessibility project
- Oversight of spending of Aiming High grant funding
- Ensuring that appropriate arrangements are in place to support multi-agency working at Tiers 2-4
- Establish links with regional Aiming High groups and integrate existing forums
- Develop an annual work plan for the group for approval by the Disability and SEN Strategy Group
Frequency of meetings
The group will meet at least six weekly
